Births
A birth is a recorded birth event for one of your patients. Every birth belongs to a patient, and each birth can carry one or more babies. There are two ways to capture one:
- the Births log’s drawer — fast, for the fields most practices track; and
- the full Record a birth course-of-care form — every registry field, section by section, with a lockable lifecycle.

The Births log
Section titled “The Births log”Open Births from the sidebar to see your practice’s births. Three actions sit above the list:
- Import births — opens the CSV import wizard under Birth Registry → Imports.
- Add patient + birth — the birth drawer, but it creates the patient inline as part of the same save, so you don’t have to add the patient first.
- Add birth — the primary action. Opens the birth drawer for an existing patient.
All three are available to roles that can manage records; a read-only Viewer sees the log but not these actions. Each row’s ⋯ menu holds Print (for every role), and Edit and Delete for roles that can manage records.
The Birth # column
Section titled “The Birth # column”The first column shows a completed birth’s Practice Birth #, with its Attendant number beneath. A completed birth with no birth date on file sits outside the counted sequence and reads NOT COUNTED. A birth that isn’t completed yet has no number: the cell shows Due and its expected due date (for example Due Jan 12, 2027), or its status when no due date is on file.
Below the selection panel, a Find a birth box narrows the log to one mother’s births — by name, the first few letters of one, or record number, the same rules the patient roster uses. The CSV, Excel, PDF and Print buttons honour the search, so what you download is what you see. The log’s starters are Last 3 months and Missing required fields; saved filters, Copy link and Print selected are covered on Filters & selections.
Record a birth from the drawer
Section titled “Record a birth from the drawer”Choose Add birth. A drawer slides in from the right. Fill in what you track:
- Patient — search by name, the first few letters of one, or record number, and pick the mother’s record. If she isn’t on your roster yet, choose Patient not listed? Add patient to switch the drawer into Add patient + birth mode without starting over. In that mode you fill First name, Last name, Date of birth and Record number instead, and the patient is created on save; Choose an existing patient instead switches back.
- Attending provider — or leave it Unassigned.
- Date & time of the birth.
- Status.
- Expected due date — the pregnant-roster’s “who is due when” datum, independent of the birth date.
- Labor timeline (optional) — Labor onset, Full dilation and Placenta delivered as real date-times. The birth time itself is the Date & time field above.
- Site — where the birth took place.
- Delivery mode — or Not recorded.
- Antepartum transfer — leave it on None — cared for through delivery unless the pregnancy left your care before delivery; setting it is what puts the pregnancy on the Transfers list.
- Birth information — optional clinical detail: Presentation, ROM method, ROM time, TOLAC attempted, Parity (P) and Gravidity (G), plus Cesarean type when the delivery mode is cesarean. Below them the drawer shows NTSV (derived) — Yes or No for nulliparous, term (≥37 weeks), singleton, cephalic — computed from these fields and the baby’s gestational age. Nothing is entered for it.

Choose Save birth (Save patient & birth in Add patient + birth mode). To correct a birth later, choose Edit from its row’s ⋯ menu.
Saving a new birth from the drawer also creates a linked course of care on the Records page in the same step, so you don’t enter the birth twice. That record starts as a Draft, and drafts are left out of registry rates, benchmarks and exports. Your dashboard’s birth counts come from the Births log, and a birth counts there once it is completed.
Possible duplicate patients
Section titled “Possible duplicate patients”When you add a patient inline, the drawer checks your roster first. If the name or record number matches someone already there, the save stops and the drawer says This patient may already be on your roster — the name or record number matches, listing each match with its date of birth, record number and birth count. Choose Use this patient to attach the birth to that chart instead, or Create new patient anyway if this really is someone new.
Multiples
Section titled “Multiples”For twins or higher-order multiples, add each baby to the same birth rather than creating separate births. In the Babies section of the drawer, choose Add baby for each child. Each baby has its own Name, Sex, Weight (g), Length (cm), APGAR 1 min / 5 min / 10 min, and Gestational age in weeks and days. Keeping every baby on one birth event keeps the event accurate and your counts correct.
The completion gate
Section titled “The completion gate”A birth’s Status can move to completed only once the record carries the fields a completed birth requires. If any are missing, the save is refused and the drawer tells you which one — in the same words the app uses:
- Attending provider — “Select the attending provider before completing this birth.”
- Date & time — “Enter the birth date & time before completing this birth.”
- Delivery mode — “Record the delivery mode before completing this birth.”
- At least one baby, with its sex — “Add at least one baby, with its sex, before completing this birth.”
- Each baby’s sex — “Record the infant’s sex before completing this birth.” (Unknown is an accepted answer; a blank is not.)
A draft that isn’t complete yet saves fine — the gate applies only to completing the record. It also doesn’t apply when an Antepartum transfer is set: a pregnancy that left your care before delivery has no birth details to record.
The “Review them” banner
Section titled “The “Review them” banner”Records that were completed before this rule was in force can be missing a now-required field. They still count toward your birth totals, so rather than drop them (which would quietly change every rate), the Births log surfaces them in an amber banner with a count and a Review them button that jumps straight to the list of exactly those records.
Birth numbers
Section titled “Birth numbers”When you edit a completed birth, the drawer shows two identifiers:
- Practice Birth # — the record’s number within your practice.
- Attendant Birth # — its number for the attending provider.
They are assigned by the app when the birth is completed; you don’t enter them. Editing a birth that isn’t completed yet shows Birth numbers are assigned when this birth is completed. instead.

A birth saved from the drawer lands in both places: the Births log row and a linked course of care on Records. The course-of-care form exposes every registry field and the Draft → Enrolled → Complete → Locked lifecycle.
