Custom fields
Custom fields let your practice capture data the standard record forms don’t already cover — anything specific to how your practice works. You define them once, and providers fill them in on records.
Add a custom field
Section titled “Add a custom field”Open Practice → Custom fields in the left rail, then choose Add field to open the Add custom field form. Each field has:
-
Label — what providers see on the form.
-
Section — which part of the record the field belongs to: Patient, Birth, Baby, Procedure or Postpartum encounter.
-
Field type — one of:
- Short text — a single line.
- Long text — a paragraph.
- Choose one — providers pick a single option from a list.
- Choose many — providers pick any number of options from a list.
- Checkbox — a yes/no tick.
For Choose one and Choose many, add the options providers can pick from with Add option. Each field needs at least one option, and options must be distinct.
Choose Add field at the foot of the form to save it, or Save changes when editing an existing one.
Your practice can have up to 20 active custom fields; the page shows how many are in use (for example, “12 of 20 fields in use”). Once you reach the limit you’ll see Field limit reached — Deactivate or remove a field you no longer use to free up room. Deactivated fields don’t count toward the limit.
Where custom fields appear
Section titled “Where custom fields appear”Custom fields show up on the record forms where providers enter them, and on the printed record.
Manage existing fields
Section titled “Manage existing fields”From the Custom fields page you can:
- Deactivate a field to stop it appearing on new records without losing the values already recorded, and Activate it again later.
- Reorder fields with the move-up and move-down controls.
- Remove field to delete it entirely. Existing values on records are removed with it, so remove a field only when you’re sure.
